I equipped a Rapier to Shadowheart and she still gets +3 to hit and 1d8+2 damage even though it clearly says she isn't proficient. She should only get +2 to hit for Dex and no plus to attack. The same also happened no matter what item I equipped to her.

This bug was posted earlier, but I noted that Shadowheart was not using her dexterity but her strength when wielding a finesse weapon (short sword in that case). So, I'm not sure what the underlying problem is. She's either getting her Dex or Proficiency Bonus in your game. What about your other characters, are they working correctly?

I've just tried it. It does applied correctly the +2 DEX modifier but not the proficiency (no additional +2 applied).
Was there any other effect on her or the rapier? Was it the Rapier+1?

Yeah. Replaying again, and proficiencies don't work at all. Equipped a shield with a battleaxe to my druid, it says she isn't proficient in the battleaxe, but she gets +4 to hit. Equipped a battleaxe to my sorcerer. Same result. Even though they aren't proficient, they can wield anything they want without penalty.

Just wanted to bring this back to light in case it might be forgotten by the devs. It's really important to the game to fix this issue.

And PLEASE don't forget scrolls. They can't be cast by everyone. Each spell has its own classes that it is listed under. ONLY if your class has that spell can you cast it using a scroll. This is just as important to the game as proficiency.

I'm REALLY hoping you guys are working on this kind of stuff. Not sure why you haven't fixed it for EA, but if it's at least in full release...

it's just UI bug the whole thing is buging all the time. But if you test it and look at the log you will see it works fine in combat at least for me.

NOTE: ranger dualing +2 damage only applies to strength, even though the PHB does not specify and should be applied to finesse as well.
