In a recent fight inside Shattered Sanctum Lae'zel was hit and fell to her death, unavailable to the rest of the party. They managed to pull back and get a full rest, but she was still missing. Had another go, but lost the fight and got everyone killed. So, what I wonder, is there any way to replace her and fight with a full party, except for going back to an older save?

There are five Companions and you can only use three at one time, so you should have two spares to chose from ... ?
That is, if you recruited them and have them waiting at the Camp, that is.
What am I missing about your issue?

In the past, when party members fell into an 'instant death pit' their bodies would appear in camp. You could raise them via a scroll there. However, someone mentioned that you could use the revivify scroll near the spot where they fell to their deaths, if there was no body available to use the scroll on. In the absence of a body, or the alternative above, I'm not sure what you can do besides reloading an earlier save.

when you hit the beach back at the game start there was a door there... inside that place is an undead guy you can talk to

assuming you got that, yes return to camp and ask to have your party member raised... he / she will keep any gear if raised this way

When a character gets instantly killed and his body is gone (e.g. by falling into a chasm or teleporting out of the map) there should be a blueish orb somewhere nearby. If you cast revivify on that orb, it should restore the character. (But to be honest I never tried if that works when you already left the area and are comming back later.)
