so I started a romance with Karlach and did the heart upgrade thing, exhausted all dialogue options and when reaching Baldurs Gate in act 3 in camp she mentions that the romance isn't continuing "I thought we had something for a minute there" insinuating it stopped which I'm confused about as she approves of nearly every choice I made yet apparently its randomly cut off? genuinely confused here

Just a guess here, but if you recall back in the druid Grove? Dammon, the Smith that fixed her heart? he said you need to find him in Baldur's Gate city. So, I'm assuming, you need to find him to continue her Quest. (Btw....since ya are in ACT3, Ya should have more of those Infernal Metal for her Quest on you, you gonna need them cause the Smith said to pick them up and bring them to BG).

I met Dammon in the Last Light Inn and spoke to him there and he upgraded it there but dont have any infernal metal, can u get em in act 3 or am I screwed?

well...I haven't gotten past ACT1, so I don't know.

In my experience, the Karlach romance was very buggy and prone to getting stuck and not-progressing.

You can get alot of infernal iron in BG city by fighting golems,, without karlach in the party (she left me in Act 1) I wish there were other uses for it.

you have to do the "the hellions heart" first so she can 'touch' you without killing you... thats doable in act 2 which makes the romance workable by then

the quest line is a little buggy but thats down to Larian skipping testing and all the camp talks jumping in on top of each other so the best work around is just rest 3-4 times in a row to get it all out of the way
