As I understand, reactions trigger whenever their specified requirement is met (on attack, when someone moves out of range etc.) and you only get one reaction per turn, which replenishes at the start of your turn. So can reactions trigger on reactions? Looking at a list of reactions, there seem to be a few that should trigger each other. For example, can you Shield a Hellish Rebuke. Can you Counterspell a Counterspell?

In regular pnp you can, but in BG3 I've had individual reactions not trigger when they should (specifically warding flare), and I've never attempted a reaction chain like counterspelling counterspell yet.
