When you wild shape Jaheira into a fire myrmidon, then use Myrmidon’s Immolation to give her haste, and then you attack, you immediately lose the haste and gain the lethargic condition.
Of note, casting haste on the fire myrmidon seems to work without the haste breaking prematurely and abruptly on attacking. Only the haste from Myrmidon's Immolation is bugged.
