My party just defeated Z'rell in Moonrise Towers. Jaheira auto-joined before the fight. After the fight, we went to camp for a long rest. Jaheira stayed behind, saying that she will wait on our return.
On our return, indeed Jaheira was waiting. Before we could speak to her, she ran ahead and got engaged in a fight with a remaining warrior. In that fight she shifts into a panther. Then she ran a little further and just stood there. After the fight, she didn't turn back into her human form.
I replayed this part a few times. It seems not possible to prevent Jaheira from changing into a panther. She doesn't rejoin the party, there's no interaction at all. Shouldn't she join again before the fight with Ketheric Thorm?

I don't believe Jaheira is intended to join the Ketheric Thorm fight unless she's in your party at the time; when I played it after clearing the ground floor she says she'll stay to secure the area, leaving my party to go up alone. This is with the Nightsong on our side though, so it could play out differently if you don't have her.
That said, you don't want Jaheira fighting alongside you as she seems really determined to get herself killed; honestly you're lucky she survived to the point you reached, I had to put her in Otiluke's Resilient Sphere to keep her from being curb stomped. =P

In my playthrough ...
Jaheira didn't join my party for the Z'rell fight, though she and her Harpers were there in the combat they were all under AI control.
After the fight, I spoke to her, and established she'd like to join the fight with Ketheric, at which point she joined as a kind of summon (ie attached to my PC rather than a full party member).
But then I went down to the basement, and she said Ketheric wouldn't fall on his own sword and would rejoin when I was ready to fight.
After I'd cleared the basement (I think, though I was hoping to see Florick down there and didn't!) I went back up and spoke to her and she reattached herself to my party.
After the fight with Ketheric on the rooftop, she detached from my party and sent me after him. I was able to ask her if she wanted to come and she said yes, but that I already had enough people. I'm assuming that this is the point at which she'd actually join as a full party member, but I didn't dismiss anyone so went down the tower and finished off Ketheric without her.
I didn't long rest between starting the assault on Moonrise Towers and defeating Ketheric on the rooftop.
I hope it's helpful to compare that with your own game.

So it isn't necessary to have her in your party. I thought I might as well try it then, and indeed, we managed to beat Ketheric without her and the story seems to continue.
