It's been a low-level issue my whole run. I sneak, pickpocket or steal and succeed on all my checks, but a guard still shows up suspicious of me. So far I've been able to ignore it.

But just now I was in Grymforge and talking to Elder Brithvar and Stone Guard Kur, and they asked me to destroy the scrying eye orb. Well, after the conversation I notice that the eye is right next to me, and there's no other NPCs nearby besides the one that just asked me to kill it. So I attack it and kill it, and immediately Stone Guard Kur accuses me of a crime and I have to pass a Deception check to get out of it. She was just part of a conversation asking me to kill it!

Just another inconsistency. There are a few here and there. Pretend, it was because the Guard needs to act out in case someone's watching.

Another favourite of mine, was when I escaped from a prison in Moonrise and got a "fugitive" tag - I teleported to Last Light for refuge, and only to be recognised as a fugitive by harpers and imprisoned in a local prison laezelmad

haha, yeah, BG3 story and systems don't go well together at times - that can be a real problem. The benefit of systemic design, is that one can come up with systemic, logical solutions. However, BG3 seems to require a lot of manual scripting to work, and I always struggle to figure out how the game will react to my actions. Sometimes you will get a magical moment, when you try to do something and it works - but just as often it tends to end up in a disappointemnt.
