Druid's shapeshifting or warlock's turn into gas spells or any other thing that shapeshifts you breaks your entire equipped items; once you un-shapeshift all the passive bonuses don't re-apply until you re-equip all your equipped gear.
The issue with druids has been known from early access, though it certainly can't hurt for more people to report it as hopefully that will bump it up the priority list for fixing. If you haven't already, then you can report these bugs to Larian via their support website. The Report a Bug link at the top of the launcher will take you to the right place.

No, it would be a fallacy to assume that Larian does or doesn't read these forums, apparently. Of course, it's also true that since release, the forums are pretty unreliable (load).
