A few key things I've been dealing with:
1) NPC's are freaking out constantly over player summons — even in places where many of them have summons or minions of their own. I don't see much of a gameplay benefit to this beyond forcing a player to micromanage their minions away from significant zones of commerce and further bugging out potential conversations as NPC's leave their established areas to try to shoo away my single dinky skeleton.
2) Many NPC's behave as if they have a scrying eye and can see everything I'm doing despite being a floor below me or completely out of sight. I was at one point invisible and entered a "restricted" zone, picked up what I could, and the second my invisibility wore off an NPC teleported upstairs and demanded that I leave. Which leads into problem 3:
3) Passing dialogue checks against NPC's when you're caught does nothing. I've passed checks to convince an NPC I belong in a restricted zone multiple times and they immediately badger me again demanding I leave or pay a fine, which ignores the previous check I just successfully completed.
4) Using items that put a guard to sleep don't work. You can't knock patrols out to stealthily get around an area because the prompt "you've assaulted a guard" shows up even if you successfully put them to sleep with a potion or drow poison. If there was another guard around to witness me doing that it would at least make sense, but I'm doing this to a single lone target with no other witnesses around.
My best guess here is their AI is completely ignoring the Z axis but I have no idea. All I can say from experience is that the game doesn't seem to care if you're on a different floor or separate room — you're still being counted as within a guard's cone of vision anyway. This makes sneaking around or stealing things incredibly frustrating and at times nearly impossible.
