What is required to create permanent potions? (skill level etc.)

And can you do this anytime or is it a once off thing?

I believe it is a one time only issue in general, but there is supposed to be a small chance of being able to buy "holy water" from the merchants. Though that seems to be a small chance indeed, I have never had the good fortune anyway.
Holy water is the item needed to make permanent potions and you need 4 skill levels to make them/it.
