Here is a system recommendation:
It seems that only quest items are important to transfer.
For quest items,2 explicit Item Flags: "Personal" and "Used".
Personal Quest Items: Quest items that are used for specific character related storylines stay with that character, always. Unless that character is dead.
Used Quest Items: Quest items that have been used for thier intended purpose (plot point, cinemeatic, object interaction) and are no longer transefered by default.
The above two flags create an implicit 3rd possibility. Unused Generic Quest Items.
Unused Generic Quest Items: This accounts for most Quest Items. These Items transfer to the player character when a companion leaves the group.

When a companion leaves the group search their inventory and containers in that inventory. Transfer the following items to the player character
* Unused Generic Quest Items
* Personal Quest Items, IF the companion that owns the item is dead

Other Thoughts:
Gold, gems, alchemy items, food, don't need to be transfered when the character leaves the party.
Gold and Keys could probably be made into an account/playthrough inventory, and not be personally held by any character.
