How the hell do you save Devella in Figaro's Pennygood shop?

If you stealth, he dies, if you announce yourself Devella dies, because every single f-ing enemy attacks her first, before you even get the chance to act as the Initiative in the game is utter BS.

Check the guides online to see if I have missed something and all they say is heal her... How the F am I supposed to heal her when she dies to before I get to act?

Everything is this game is just poor, it's just there to p*** you off.

I do not agree with your last statement.

First engage the encounter while concealed (crouched is sufficient). Second, choose the dialogue option that most directly causes you to intervene. This may simple be the "attack" option.

Those two steps "should" create a condition that allows you to act prior the hostile npc no matter what your stats are or how the initiative rolls land. You can increase the chance of this by ungrouping the other members of the party and parking each one of them close enough to be able to respond yet out of the way prior to activating the encounter.

When the suggested method and tips for resolving a situation involve both exploiting meta-knowledge of the situation and abusing the game's out-of-time non-responsive world conversation locking to position characters advantageously while everyone else stands around like stumps watching because you're in conversation... then it's very hard to argue that this isn't straight up poor design, and suggesting otherwise is pretty disingenuous.

To be clear. The scenery of this encounter already suggests that you are walking in on it with the opportunity to "surprise" if you respond quickly and decisively. It does not require the meta-knowledge. What I offered was a way to increase the likelihood that the encounter ended with the desired result. What may really cook your goose is that not only is the result the op desired specifically not required, but that the encounter may never even occur within a campaign.

Options:

1) Cast Sanctuary or Invisibility, or throw an invis pot on Devella.

2) Having Alert as a Feat is really useful for situations like this. Initiative is king in bg3.

3) Otherwise Elixir of Vigilance is really good because it should put you in top of turn order.

4) OR if you have met with devella before and you got max rep with him by giving him Gold then you can cast feign Death.

5) You can also respec Jaheira as a Monk 8/Rogue Thief 4 as God intended and just murder everything before it has time to act, because Monks are crazy OP, have insane movement, and with the right gear can do 250-300 damage per round hasted.
