I love this game and am on my I think 5h playthrough. But I just encountered again something that if I had one wish for the game I'd change.

It's so irritating that, despite the fact you're psychically connected to them, companions like Wyll or Karlach can't understand you are lying to Minthara to gain their trust and don't intend to do bad things for them.

Losing approval because you are playing a con is very irritating.

If it's any worth their approval don't matter much in romance/playthrough/personal quests, if you do a good playthrough. I never even have them in my party and they still try to jump on me, at least Karlach understands what a 'no' means.

The easiest way to correct and account for this, as Dex says, is with the dialogue choices being a tool lets you communicate, player-to-game, what your intention is so that the game can respond appropriately in universe. They don't do this, but they could.

I am afraid that some things simply are not possible in a cRPG versus pnp RPG.
I had the same issue when I deceitfuly offered to buy Gnome slaves, or that hentai guy in the Zentarim store.

When I played shadowheart I used [Deception] dialogue choices whenever possible (aspiring to be Dark Justiciar and Shar's chosen...) Would [Lie] choices be something else?

Tbh, I don't remember if companions reacted properly to those dialogue choices or not.

They understand deception, but sometimes we don't get deception options in dialogue where we really should.

I'd agree, a deception option or something flagged as lie that you need to pass a check on. I know it's not a big hit to approval but just annoying.

If the companions would be in your head always, then you couldn't for example hide the body of you know who in the Durge story - as I understand, you have to activate that connection and some experiences are shared. You can for example share with Lae'zel, why you did let the Guardian live in teh creche scene, otehrwise, she doesn't know, why.

However, there could be an option in dialogue to signal them, that your are about to deceive someone.
