Sorry about the awkward thread-title, but this is one is a bit hard to describe in few words.
I *think* it started with Patch 4 for me - I hadn't noticed it before. Plus it seems to be a Shadowheart-exclusive thing, because it will only happen with her. It happens very consistently with her, but, so far, not with any of my other companions. I'm currently in Act "1.5", I'd say ... done the Druid Grove and entered parts of the Underdark, but haven't done the Monastery yet, for example.
Shadowheart will somehow "lag" behind when I leave camp after a long rest. Most of the time it's her running around in her camp-clothing while in "adventuring-mode". She is set to default clothing mode (and the icon in the inventory shows that too). What happens is that, visually, she will still be in her off-hours clothes after a long rest and I have to click on the clothing-mode icon to put her back into her armor.
This isn't just the clothes, I believe, but sometimes she will also not show the exclamation-point bubble when she has new dialogue available at camp. Instead, that bubble will only appear the instant the party leaves camp after a long rest. Since this isn't my first play-through, I'm very sure these dialogues of hers have triggered in camp in the past for me, not right after leaving camp.
It's as if she is not "getting the message" somehow that the party has ended the day/has left camp after long resting. As stated above: None of the other companions have acted in this way (yet?) on my current play-through... neither with the clothes-thing nor with the "delayed dialogue cue"-thing.
