Happened last night. I was at camp in the Monastery-region and I had just finished the whole Creche-arc.
Ended the day successfully and rested. Then, in the morning, didn't use "leave camp" but instead tried to fast-travel directly from camp to the Myconid colony - which resulted in the "you're traveling to a different region"-dialogue. Clicked "Yes" but instead of the game loading the new map, the scene with Voss visiting the party at camp started to play and my Tav was in his off-duty clothes again. Basically, as if I had just attempted another long rest right after waking up from the last one.
The scene played out normally, but once it was done, it was night again and I had to take another long rest (no prompt to use camp supplies, though). When everyone woke up (again

), I clicked "leave camp" and the game did load the new map and put me into the Myconid colony.
Unrelated but since it happened during the Vlaakith-encounter and I don't want to open another thread for this:
During the dialogue with the Gith queen at the creche, my entire party was engulfed in some sort of weird fire-looking effect. Didn't look like any regular effect I've seen in the game so far, more like a perma-looped, glitched version of the elixir visual effect you get on some characters every time you switching back from dialogue-mode to regular world view. No other characters in the scene were affected and it did go away when the dialogue sequence had ended.
