Shadowheart keeps falling to her death everytime she goes up and down on the final lift disk to the Shadowfell. She just slides right off. There was a similar issue with some of the other characters on the lift at the Grove.
Could you try switching to her before you get on the disk? I don’t know if it would make any difference if you were controlling her. That is a really bad bug. Definitely send in a report.
