After patch 5, i see 2 issues when trying to get epilogue:
- I dont get the epilogue
- I dont have my Karlach avernus scene
- I dont have the "romance" scene with Shadowheart

I had the last 2 when i ended the game a month ago

I tried "everything", uninstall game, verify from steam, starting the game from the following save points:
- Before talking to voss about saving orpheus
- In the sewers, before using the "no return" point
- Before the Huge fight / after talking with every allies who joins you in final battle
- Before climbing in the brain (so before the last last fight)
- Before last hitting the last boss
- When i can betray orpheus or let him kill the brain
- Lastly, when we can convince Karlach to quickly go to avernus


Also, i tried those case with and without mods, doesnt change

In all those case, it goes directly to "congrats you save BG" cinematic, then credits, then post credits as usual

I already sent a bug report to larian, but i was hoping, some people here had solution

This is the answer.
You used mods on your profile, then turned them off while thinking that it is enough. But it is not. Since mods could be made by anyone, both professionals and amateurs, they can bring instability in many unpredictable ways. Many of them also affect the save files.
And it is important to understand, that the mods for previous patches and save files with those mods are not compatible with the new patch(!). With any newer patches.
This is the downside of using mods.
I also want to remind you that many modders write in the instructions for installing mods about the need to disable telemetry. And they are quite correct about that. That's because data from user is not representative anymore, while contains errors from mods.
If you want to play the new patch without problems, then you should remove completely all the old mods (clear mods folder) and all soft for mods (like Script Extender, all files it's creates, etcetera) and start a new game. You will be able to gradually restore your mods when the authors update them for a new patch.

In my case, getting the epilogue depended on my choices in gameend.

Your very first choice leads to the Karlach's phrase "In my name" - Credits - Main Menu
