Basically its a mode that can be used alongside any difficulty but adds more story and gameplay consequences for using the tadpole and the amount of tadpoles you consume.
Story related
One of the first things I thought of it would be a good excuse in bringing back Daisy from EA as a separate character alongside the dream guardian as a result of using the tadpole like it was in EA. this will also make the songs the power and down by the river relevent to the story again.
More opportunities to use Illithid persuasion in later acts like in act 1 but will have a consequence where it will reduce normal persuasion by -2 and this effect will go after a couple long rests. for example this will be handy when your overwhelmed with guards and you use illithid persuasion to get out of that situation.
Urges like the Durge but because of madness rather then being a Bhaalspawn. For example in Epilogue where the Mindflayer character has a urge to eat the other companions brains.
Gameplay related
For obvious reasons this toggle cannot be disabled but difficulty can be changed lets just call it a illithid Madness enabled save.
Create a custom Daisy alongside the Dream guardian
Buffed Illithid powers but has the chance when you use them to attack other party members (as you temporarily become a slave to the grand design).
Less resistance to psionic attacks made by mind flayers.
Omeluum's ring reduces the chance(dependant on difficulty) for your Illithid powers to cause your character to attack your companions.
