I'm using this guide:

Sometimes, I get no approval despite the party being near enough. Example: anything related to Benryn and the flaming fist. I've reloaded multiple times, but it's just not happening.

Occassionally it's just one character, but most often it's all of them.

Yeah this has happened to me multiple times in the game when doing the Companion approval stuff. Not just you.

Damn. I hope they fix it. I've encountered it again today with the Zhentarim. I might be short 10 approval on basically all companions already.
