I love this game, and I love Act 3 in particular, though if I'm being honest it also feels the most unfinished. Certain quests seem to end abruptly, or feel like there are natural alternative outcomes that don't seem to appear. I hope that Larian has plans to round some of these quests out, and given that they've already added the epilogue party and all that, it doesn't seem unrealistic to have some hope.
Felogyr's Fireworks - this is the first quest I encountered where I openly said "that's it?" The early part in rivington feels fleshed out, but once you get to the building in the inner city there isn't much to it. No NPC will ever let you to the third floor, and as far as I can tell the only option is to fight everyone in the place (and probably the fists too). I feel like I should be able to infiltrate them temporarily (especially if allying with Gortash) and for an evil playthrough somehow ensure more bombings go off in the city.
Mol - I know I'm not the only one to complain about this. I feel like we should at least be able to reunite her with the other tiefling kids. Maybe sneak them into the city somehow? It also feels like the contract with Raphael really goes nowhere.
Githyanki Egg - I really wish there was a more morally good outcome for the egg. I was really hoping I'd run into a couple who wanted children but couldn't conceive, or maybe some Githyanki that had defected from Vlaakith trying to make a living in the city. I was also surprised to see Lae'zel had nothing to say when we ran into a Gith murder savant after selling the egg to the society of brilliance back in act 1.
Take down the Foundry even if you kill Gortash first - Not really a quest but I don't see why we can't take down the foundry and the watch if you kill Gortash first. The Absolute would likely still want to keep the steel foundry operating for her own ends. The quest could largely remain untouched. Just let me do it please Larian I just want Gontr Mael still
Stop the Presses - In my evil playthrough I slaughtered J Jonah Jameson and nearly everyone else in the place, and the quest still seemed to think I needed to mess with the machine to stop the news. The papers also still kept coming out, which feels impossible when there's no one left alive to keep writing for the Baldur's Mouth.
Carm's Garms After her brother's death - Not a quest the game gives you, but there's a note in the basement of Figaro's house that suggests that Carmen was supposed to inherit the family fortune, yet it looks like she's in a poorer part of town. In my evil playthrough I killed Figaro, hoping that she might move into that shop after his death, or perhaps upgrade her own shop. Maybe Naaber gets a job as her assistant. I'll admit this one might be a little much I just really like Carmen as a character.
Steal the stolen money back from the Zhentarim - If you let the zhentarim take over, it makes sense that Roah couldn't be convinced to give back the money, but I don't see why I couldn't steal it back from her.
Letting the Hag keep Mayrina - In my evil playthrough I let the hag keep the girl and she never showed up again, but Ethel still ate the other woman's child. Did Mayrina just die or something? What about her husband? Ethel seems keen on keeping her side of the bargain, even if it turns out to be a curse.
These are just the ones I can think of off the top of my head. I'm sure there are plenty other quest outcomes that could ideally use a little more fleshing out.

I agree with those. I'd especially want to investigate the firework shop more, show Carm the real testament or find some use for the blackmailing material from Cazador's vault, for example you could give it to Politician-Wyll or his father, at least. The Githyanki Egg has a new ending option with Patch 5 though.

Oh that! Thank you for reminding me, I forgot to file a bug report that. Apart from not reacting to his portrait, Oskar was supposed to paint Astarion but did paint Jaheira. =D

Huh. Well. I agree that the third act feels the least 'finished', but at the same time I rather like the freedom of running around the city and finding new things everytime. I'm almost at 700 hours into this game and I keep finding new things or different outcomes. And, I am rather content with the sidequests, really. I have to think real hard on one. I quite like how they are handled, and how some small bread crumbs lead to big game changers [Nightsong, Ironhand Gnomes] or highly rewarding endings [Book of Thay, Minsc, Zentharim] . There are also encounters and setpieces that I *really* like. Boaal springs to mind, the Swamp Tea house is brilliant, figuring out how to grab the that holy artefact in the monastary - super fun and highly rewarding. The final assault with the allies is lots of fun, and the underwater segment with the bubble-spawns is probably my favorite bit in the whole game; it is fun mechanically and the atmosphere is great. So I have to think hard on something I didn't quite care for, or thought that needs work. But: On these forums I make it no secret that I dislike the writing for the Emperor. With that, going to look for old house feels stupid, especially since he just tells you to go there when you walk past it. The quest design itself, though is fine; with the cellar and the encounters within. Also, mostly for the same reason - Wyll's Act 3 Quest. Also because of how the quest is resolved; "Oh well, then it wasn't necessary at all." Then. The Tribunal bit with the Throne of Bhaal callbacks. Nope. I didn't like that, and they did a serious disservice to beloved characters here, one in particular.

Rescuing Omeluum from the Iron Throne feels kinda pointless to be honest.
Letting him die or live has no real repercussions. He just gives you a bag of items you can find by scavenging. The only real thing he offers is the teleport to the sub ability that allows you to instantly rescue someone, but on average, by the time you reach and free him, you only have 1 or 2 turns left, so you can realistically only use the instant safe ability on 1 person.
You can't even trade with him again after interacting with him like you can in the underdark.
In addition, why is there no option to have him meet The Emperor? Both are rebel Mind Flayers that broke away from the Elder Brain, and Omeluum is the good aligned version, and The Emperor is the Evil aligned version, so why can't we ever tell them about each other?
It feels like such a missed opportunity

BOOOAL and the Kua Toa. Either having BOOOAL show up later on as we harvest more blood for him (Perhaps he gives us new shiny things to further the bloodshed) or the Kua Toa appear bringing gifts for their new god (As well as more scenes of bloody sacrifice)
The Shadow Druids. Okay, so 3 Shadow Druids appear... Then get promptly murdered and then... This sect never appears again in the game ever, despite their being 2 Druid companions and a Druid heavy ally faction?

I agree, the shadow druid plot just somehow burned out. We have two druids in teh group, one of them with no plot whatsoever in act 3 (Halsin) and no follow up on teh shadow druids? I know, a city is not their usual haunt, but there could have been special circumstances (destroying teh enemy from within).
And Omeluum should have been the obligatory mindflayer at the end of the game, if rescued

This is mostly a thought experiment, since it's likely things were cut for story or pacing reasons. But in an ideal world, these feel incomplete to me:
1) Halsin letting Kagha off the hook for... reasons 2) Shadow Druids completely dropped. The only further mention is Halsin distaste for the city and asking if they were right 3) The dead tree and poor druid in Act 3 (can you tell I like druids?) 4) Omeluum feels unfinished. I personally disagree he should be able to suddenly make a happy ending -- he's not a fighter -- I do feel it seems like it just drops off. Part of that perhaps is the fact the Ring of Mindshielding doesn't work how he says it does. 5) Unless you are a lore junkie, you don't really know the strange ox is evil. He's a great ally, but why does risk his life for the simple favor you do him? Does he have a connection to the city? Does he really want to retire? Or does he have some ulterior motive to getting inside? 6) Mol. Anything. 7) There are a couple of refugees with lines that beg you for money, but you can't give them anything. 8) The wavemistresses as any ally. If you have already killed the submarine, you can't even tell them afterwards. A bolt of lightning from Umberlee would feel great.

I similarly encountered people in the city who I wished I could interact with. There's a tiefling child with murdered parents, and I instantly cast speak with dead on them to nothing. I similarly wished I could have helped the druid with the tree. If I'm being reasonable it's probably true that these were always meant to just be flavor text, as creating even small quests for all those citizens would be a huge amount of work. 5) Unless you are a lore junkie, you don't really know the strange ox is evil. He refused to change into his monstrous form and died in cutscene as I hacked him to bits. He literally said that he didn't want to murder, he just wanted to be an ox. Everyone talking about Omeluum- I love Omeluum and it definitely makes sense that he would also fight in the final battle, though perhaps as a 'weaker' option since he doesn't seem like the fighting type. I would also settle for him being some sort of ally summon. I think the rationale the game has is that he's not present at the moment the decision about what to do with Orpheus, and therefore can't be called in to help. Along those lines, I also wish you could volunteer any party member to become a mindflayer, maybe with a persuasion check (or having convinced them to tadpole themselves earlier). I know Karlach can become one but don't know about anyone else. Either way, it would be fascinating to see who could do it. Wyll, Gale, and Shar's Chosen Shadowheart all seem like prime candidates. Lae'zel could do it to spare Orpheus. Minthara wants to usurp gods, and could see it as her ticket. Jaheira's got a history that would make her primed to sacrifice for everyone. Minsc would just be funny.

Another one I just thought of - If you save Duke Ravengard you should be able to turn Wyrm's Rock back to ally status, especially if you sneak up and just kill Gortash instead of fighting everyone. The previous leader revoking his endorsement of a corrupt ruler should have some weight on the city.
There's also the old guy who refuses to leave his cell in Wyrm's rock. I was always so curious about him. I literally killed the guards and made the way out easy, and he just decides to sit there? At least tell me why.

Wyll has already proven he believes in sacrificing himself for the greater good. Jaheiera seems to believe the same way. Shar's Chosen Shadowheart says she doesn't know what's right and opts to blindly obey instead. Honestly I think Astarion is the hardest one to justify because he's all about being free but the others all have situations and motivations that could lead them to being a mindflayer.
