If the player kills Karlach for Wyll and dismisses him from the party after killing her, the game will have the Dark Urge behave as the 1st companion in the party when Mizora arrives to the camp.

In this example, my poor traumatized Dark Urge believes she's Shadowheart;

It's not just Durge.

I've had this happen a lot and not just with Mizora.

I will be in control of my created character be it Tav or Dark Urge and then I will get a cutscene where I am talking to someone but it switches to say Karlach or Astarion or Shadowheart even though I am in control of my own player made character.

So this is a persistent bug in various different ways.
