I went to the surgeon in the shadowlands and I convinced the Surgeon to have his nurses kill each other before I started combat. I killed him before he could do surgery on my party since he did not get a surgical tool to use on any of my characters.

Anyone have any idea why this is not popping up? I think this is bugged and I am on the Xbox.

Does that achievement mean that you kill him before he attacks you at all? I have that achievement, can’t quite remember how I got it, but it was something like that. Hopefully others can confirm or deny.

Maybe the achievement, like Ecc2ca says, is about not fighting him at all? There are two ways for the party to kill him without combat.
I also have this achievement but I don't remember which of those two routes I had taken in that playthrough.

It's not a bug. If you're not scrupulous about reloading, you can replay the confrontation and see if there are other ways to resolve the situation.

No, I think I got the achievement by killing him before he took a turn? If you have high initiative, you can go first, find a way to incapacitate him, and then just HAMMER him. Lae’zel should help with that and also maybe some scrolls from the trader lady in Last Light.

Yeah everything online states that you are to kill him before he can actually perform the surgery attack on you with a surgical implement which he is supposed to get from his nurses. You have to actually be in combat for this achievement to happen. I checked my combat log and there was never a time he performed that skill because I kept his nurses dead and hammered him with Karlach and Gale. Karlach with her barbarian rage and double attack and Gale with fireballs. Astarion provided ranged support with sneak attack damage and my main just hit him with magic missiles.

Yeah, it could very well be bugged. Because what you’re saying is true. If the nurses are dead, then he wouldn’t have anybody to hand him surgery tools, right? I’d probably try killing him before he can even attack (in the case that he has another tool on his person or something) and then, if that doesn’t work… that would be an annoying bug.
