basically, in ACT II when I go with the nightsong in shadowheart's story, when I confront her, she doesn't leave me any dialogue option, shadowheart simply kills the nightsong directly, I have maximum approval with her, all her dialogues raised doubts about Shar and indicated his past to Selune, this has only happened to me in MULTIPLAYER, since in my solo games this problem has never arisen, any ideas?

I have never had this issue either. I’ve only played that scene in single player, but probably five or six times. This sounds like an enormous multiplayer bug. You should always be getting that persuasion check or some kind of conversation with her. Definitely should report that!

By default, Shadowheart should always choose to act upon her subconsciousness of being a Selune follower (she was only magically made by Viconia to follow Shar via memory wipe and 40 year's indoctrination). Her "default" choice is always to spare Nightsong.

This has nothing to do with her approval or your relationship with her.

Nightsong is a most critical turning point in Shadowheart's story. If Nightsong is not spared, Shadowheart will never be able to recover her memory and will forever remain a faithful follower of Shar. However, the subconscious version of Shadowheart will always force Shadowheart to spare Nightsong without even understanding why (due to memory wipe). Nightsong is the only means/entity in the game that enables Shadowheart to set upon the path for recovering her lost memory.

No idea what happened in your game. Are you surely you didn't try to "influence" her earning her approval?

Even in my durge run, SH only killed NS when I encouraged her to do it. Left to her own choice, she spared Dame A.
But I 've seen a case where the TAV had died in the battle with Balthazar. In a party of 3 without Tav, SH resolutely and unquestioningly kills NS.
Maybe something has triggered the game to think there's no Tav.

You have an interesting case. I don’t know what actually happened, as I don’t play as a fully customized character Tav. However, I have a hypothesis here. With Tav dead, the “remain silent” option is also gone. Shadowheart will act on Tav’s default action, which is unlikely “remain silent” - hence she’s determined to carry out her mission. Make sense?

I found the video back that shows the case. Putting the rest in spoilers.

Without Tav there can obviously be no conversation before SH takes action, so like in your case she confronts Dame A. directly. Now there are still 2 options shown in the vid. If SH carries Shar's spear, she execute Dame A. But you have the option to make her throw away her spear before confronting Dame A. And then she is spared. If you have a save to reproduce your case you might try to see if you can throw the spear away also.
