From the perspective of the current version of the game there is no in game reason why his face is disfigured and from what I seen online there is no reference of it in the current game in books or cinematics. but what I have seen online there was cut content which explains how his face got disfigured.

I suggest three fixes for this.

1. Restore his act 1 appearance (simple and quick fix)
2. Give a simple mention for reason behind it for example it was a punishment made by a kithrak or inquisitor loyal to vlakkith which get referenced in sharesses caress.
3. Restore the content (if it exists) associated with it in a definitive edition or dlc.

I'm more than happy for extra content, but that aside, I think the wounds he's suffered since the last time he was seen is great. It gives the sense that he's been through some real trouble.

Those scars speak without the need for exposition.

Didn't he even say, that he ran into some trouble? I don't think, there needs to be more explanation.

And he gave up his protection device to help the group, so it makes sense that he himself is now more vulnerable.

Agree with previous posters, it's fine as it is. I thought it was a cool detail and that he must've gotten seriously attacked at one point since he's now openly against Vlaakith.

The inquisition was also already suspecting that Voss is a hshar'lak (traitor) before the player even meets him at the camp, since there is a letter on the left side of the room where the Inquisitor can be found, so the clues pretty much imply he ran into the inquisition just like the party has on their way to Baldur's Gate;
