meant to post this a little while ago but on one specific save shadowheart's icon is completely messed up

I play on PS5 and sometimes when I play a Orc there is a black gash on my portrait.

do you have a picture of it? i'm curious
sometimes when i'm playing a tav their icon on the inspiration screen is completely default, but the shadowheart thing is confusing to me because it only happened once on said save file and is stuck that way (i'm guessing until i remove her from the party or some other trigger)
