i'm sure i'm missing something completely obvious here, but how do you get the teleporter to point to the third area?

When you are on the eastern island, drop a teleporter stone there, return to the mainland using the triangular teleporter platform, then use the other teleporter stone to get to the island. You can also toss a teleporter stone over to the eastern island to get there.
