After installing the hotfix, I'm having trouble getting my native resolution 1920x1080 to work. When I select that resolution in the options, I'm getting black bars on either side of the screen as if the game is limiting the aspect ratio to 16:10 instead of the proper 16:9.

If I check the "use desktop resolution" option, it fills up my screen, but then stretches the 16:10 image to 16:9 resulting in slight artifacting from pixel stretching.

Anyway to fix this? I love the unlimited FPS fix and really don't want to lose it if I have to undo the hotfix!

In Windows Explorer, try browsing to the folder below, open the 'graphicoptions.xml' file in Notepad (or other text editor) and make sure the proper resolution is listed.

Try running the game as administrator, and (in Vista/7) disable desktop composition in the compatibility options of the program/shortcut.
