I just completed the quest, and I was reading a FAQ that says he should give me a Dragon Skill book, and also the completed quest text says "The librarian was grateful for the book. As a reward, he gave me a Dragon Skill book, which should one day prove to be very valuable."

Yet I never get one added to my inventory, how do I get this book?

In D2:ED you got a dragon skill book from the librarian; in DKS you do not.

Yes; in the second patch you'd think they would have restored the book or changed the conversation and quest log references to it.

Do you know of any other changes/things left in like that?

There were a couple dragon skill books (dropped by certain opponents) that got moved in DKS, but there were no references to them. I don't recall any other examples of changes in DKS mentioned in the forum (have not played it yet) which still retain descriptions from D2:ED.
