Okay, so when I played through the game originally, I only sought help when I got stuck. I made it through to the end with only missing 5 achievements (yes, I am on 360). I think that's pretty good. However, as soon as the end credits finished, I started a new game to get the achievements I missed. What I didn't expect was to see things I never saw before (aside from the things connected to the achievements). So far, I witnessed a robbery, found an enchanter I never knew existed (behind a shrine), and a wishing well that I never knew existed. I am even more glad I decided to do another playthrough and hope to find more new things. I am sure I am not the only one to experience this, right?

True. I discovered certain things in my forth playthrough that I had missed previous and I'm still sure there's things I haven't discovered quest-wise. I'm still wondering what that "Chicken Rune" does, I'm sure it has some connection to that earth mage in High Hall. There's multiple ways to finish that quest, oh and I still haven't sided ever with that Goblin Lord Sydifyrian (or whatever his name is spelled like).
In a hidden storage near Jackson's farm, there's a note behind the secret wall. It's written by Jackson, it's under a closet which contains a skeleton. I can't believe I didn't notice this in my previous play throughs (as I had visited there before) but the skeleton obviously is Booth (I gained this from reading the note which I hadn't previously done). Funnily enough, once you read Jackson's diary, the quest to confront him is called "Skeletons in the closet" and I found that pretty funny because Booth's skeleton REALLY is in a closet!!!!! XD
Thumbs up for Larian for that one, the small details they put into the game really do make it all that greater.
Oh and then there's the skeletons, notes and treasure you can find around The Battle Tower once you destroy's Damien's stupid armada!TM. There's a tomb with a note - that from what I recall - is quite cryptic. It's signed with "M" at the end which led to speculation that the skeleton in the tomb, could be that of Maxos.
Have you discovered the sleeping adventurers in Broken Valley yet? They aren't part of any quest but if you fetch them an apple, beer and healing potion (from what I recall) you can awaken them and receive a generous reward as a result. They later reveal (back in the village) that the last thing they saw before they fell asleep was a "rhyming wizard" which is obviously Bellegar...

I played ED once and then through DKS several months later. I found things I never found the first time through but I couldn't be certain if they were added in DKS.

In the conversation if you say that there must be a peaceful solution, you can use the rune, which gives the best reward. He says the only way he would leave the village in peace was if he had a rune to conjure food, so he never had to go there again.

I did discover the sleeping adventurers, although I had to jump my way up a cliff to do so. I only found them because I was trying to get to the ruins for Bellegar and couldn't find another way around. Surprisingly, I actually had what I needed to wake all three (even the apples). I also was able to scare away the fortune teller, which I didn't do the first time through (and which was connected to an achievement. I now only have one achievement left to get, and it is (obviously) for something I never found the first time through. Gotta get through Sentinel Island first, though, so we'll see what I find here.
