Okay, I've mind read, defeated and looted Kali. However, I still didn't get Ulthring's Helmet. Did I do something wrong, or miss something somewhere? Thank you in advance for all of the help you may be able to give me on this, it's greatly appreciated, I assure you.

Sounds like a glitch. There aren't any other prerequisites you could miss or things to do wrong. If you have the Developer's Cut version of the game you should be able to use the Developer Mode cheat window to give yourself the helmet.

Do you have a recent save you can reload and try mindreading and killing Kali again?
