I've noticed that the ingame Vsync option only applies in Fullscreen mode, while in Windowed and Fake Fullscreen mode it does nothing.

Since forcing Vsync through my Nvidia drivers doesn't work on games running in a window either, it's very important to me that the ingame Vsync option does the job. Otherwise I have to choose between Vsync and Fake Fullscreen mode (btw as one of those crazies with dual monitors, I cannot express how much I LOVE it when developers take the time to add a borderless window option!!)

I normally use Aero yes, however I've tried with Aero disabled as well and it makes no difference to this issue.

Actually, this time Aero should be enabled, haha. I recall a bug when windowed vsync didn't work due to disabled aero.

I don't think individual applications can enable V-Sync if they aren't full-screen, since it involves controlling the entire video buffer, does it not?

Besides which, the Windows desktop since Vista has automatically enabled V-Sync. Perhaps the screen-tearing is caused by something else?

As far as I know, issues with lack of v-sync in windowed mode are leftovers of the past, WinXP times and old d3d. With latest version of d3d9/d3d11 under win7 it is possible to have v-sync in windowed applications. I don't recall where did I read about it, though.
