to gain entrance to the rivertown place, other then lockpicking the door on the west side? And also how do you trigger the quest, can you do it before, or only after entering rivertown?

Do you mean the quarantine section? You may enter it without a quest - you'll get 3 quests in there.

Which other west door could you mean?

Also is there a quest you can get that says you should break into the quarantine secion and help them? Or do you just have to break in, then get quests?

You just break in on your own, and then get the quests. I always thought that the guards would attack me, so I missed out on them the first time through the game.

No quest to break in - just curiosity? But there is a hint from Lain, to get the key - and as the ill people are in this section, it's logical to break in.

just keep clicking on the locked front gates, and they then turn hostile. You can kill them with no rep loss too!
