Several people have had that problem with the companions and another NPC. One reported that that issue went away after they talked to Madora in the inn's cellar. Saving and reloading may also help.

Revalidating the game files in Steam also seemed to (at least temporarily) fix the problem for me - but many others have already reported this, so Larian must be working on it.

I haven't seen it again since that first time, I think the saving, quitting and reloading worked.
