Excuse my ignorance but when creating characters for the first time in multiplayer can the player that joins create his own character (eg. sharing char creation screen before start) or does the host have to set up both?

No. He'll become the character you're currently not playing.

Yes, when you first start the campaign in multiplayer the other player can join and will be able to create his/her own character while the host will create the other one. Once the creation part is over there is no changing characters when you join any host! You then will take over one character that the host is currently not using.

But keep in mind, that all data will be saved on host! You will not be able to play your created character in singleplayer and then meet up again. If you would like to join the discussion about that you can find it here:

So if say if me and my mate want to play through the whole thing together from the start, he will tell me what character he would like to play and i'll have set him up as my second character at the character creation screen?

No, start a new campain in multiplayer and have him join while the host is still in character creation screen. He then will be able to create the character himself, and you will be able to create yours, simultaneously!

If someone joins in the character creation screen, then you both customize your own characters.

If the host is already in game when you join, then you take over the main character they are not controlling.
