It's easy to miss but directly above you in that screen shot their is another teleporter hidden behind a curtain (it's visible in that shot actually) if you look at the map it should be obvious. There's also a rat and a book in that room that tell you what order you need to use them though if you have two green portals then I'm pretty sure that's all your missing
