Fumbled means you failed at something. It's (sometimes?) different than successfully *trying* something that is resisted/immune/whatever. For example, if you summon a Fire Elemental, you may see on melee that it "fumbles applying Burning" (quote from memory, may be inexact). When it does that, you won't ignite your target. Other times, you may see a target resist warm or burning from the elemental. I believe that's two different mechanics (the manual isn't helpful in this case).
Thus, whoever fumbles something fails at it, and the cause of it should be failing to make whatever % roll the effect required. Generally, you shouldn't worry too much about fumbles. If you have your mages casting magic, and your archers shooting bows, and your warriors hitting with weapons, then you're operating in your strong areas, and should only rarely fumble. If you see yourself fumbling something often, though, that's a good sign that the character isn't very good at what you're trying to have them do.
EDIT: So far I've only seen fumbles on procs; that is to say, things that are intrinsically part of something else - the aforementioned heat of a fire elemental's attack, or a weapon's chance to do something on hit. I'll play around a bit and see if I can fumble anything else.
