The title might be a bit misleading, but long story short I'm on my second playthrough. The first thing that I noticed is that when I went to Hilberheim the blizzard wasn't there (I had opted to disable it with a wish in the first playthrough). Then I noticed some of the chests that should be there, weren't there... but the quests and everything played through more or less like normal.

Now I'm at the Luculla mines and unfortunately they are covered in lava. Again this is my first time in the mines on the second play through, but it seems to have kept a lot of the environmental conditions from my first play through.

Anyway to fix?

Yeah, that really shouldn't happen...

If you mail Larian about it, you can send your save file to them with the analysis tool http://www.larian.com/OriginalSinAlpha/AnalysisTool_DOS.exe , they'll look for what's wrong here and might even be able to fix this so you can keep playing.

I had minor things carry over with an earlier version (playtime and such) and they suggested I should fully close the game before starting a new one. This might work here as well, but you probably don't want to do everything all over again.

This seems to have happened to me as well. Some monsters are already dead (such as the guardian and the immaculates in front of the fire temple in Hiberheim), and the fog of war is revealed as if I had already explored the area (has happened on the Luculla and Hiberheim maps so far, did not notice whether it happened on the Cyseal map).
