I wonder if it would be fun if very rarely our character could burn himself with a fireball or drop the weapon instead of hiting with it? Or throw a bomb to a wrong place and hit friends?

It would be a very rare event but I think it could be fun!

What do you think? Ideas to attach?

Grenades are inaccurate in the EE without a talent to allow precise targeting.

Spells backfiring were discussed in the forum for D:OS as a possible penalty for trying higher level spells with a low intelligence, etc.

No; there was quite a bit of feedback considered in balancing changes, but that idea was not implemented.
