If you installed the game recently, it should be up to date. The latest PC version (shown in the main menu, below the column of buttons with the keyboard UI, or the top right using a controller) is 2.0.119.430.

Console updates are automatic if you are online when you play the game.

No idea. The others were all 1.0x, so maybe 1.06... unless it is 1.051, since it is a PS4 specific issue.
