This one has happened a couple of times for me:
First noticed it when I decided to crash the fort and kill the guys turning the elf into a mute monk. I'd knocked down the head guy and when he'd gotten up, I found my character couldn't hit him and didn't use up any turn points when I tried. I used spells and melee attacks but nothing hit and none of the turn points were used. The only way I could end the turn was to either press space or move away.
The second time this happened was on another random fight later on and this time it was my ranger (additional character... the elf canibal... can't remember her name... think it starts with "S"... you get the picture) that couldn't hit.
