(1) If you take the Undead guy's soul jar, rather than destroying it, and return to him, he'll ask you to destroy it. Refusal means he instantly engages you, and afterwards the kid gives you an item and asks you to still be friends.

However, it appears that having this interaction leads to the Kid (Han; kid with boat) + some NPC's becoming marked as red (the neutral undead merchant next to the bridge for example).

(2) I've seen this same issue occur, if you pre-eminently attack the guards hassling Han (the kid with the boat), attacking them without hitting him will permanently mark him red, and the merchant too.

I should clarify, the kid 'Han' the one with the boat, becomes marked hostile. The others in the cave and area do not.

There were some adjustments to alignment groups, which should help fix situations like the first issue.

For the second issue, randomly attacking people can drop NPCs' opinions, possibly so they turn hostile, even if they don't necessarily like the NPCs you attacked.
