I'm replaying and every other time i've gotten the hero tag when i give Yarrow Migo's ring. This play through - no tag? I don't think I've done anything differently, any thoughts on why that might be?

Never had the hero tag on this occasion.

The only way to get Hero Tag in the beginning of Fort Joy is by defying Griff when he asks you who stole his fucking orange. Fuck off Griff.

I didn't get the Hero tag until awhile after leaving Reaper's Eye, and then I immediately received the Villain tag. /shrug

Not sure but I think you have to do multiple Good Acts to get the tag, At Fort Joy you get the tag after you solve the quest but also helping the magister on the boat while sinking.
