My friend and I wanted to get the altar achievement, and to do so we went around the island killing and looking for altars. Halfway through we found the Sallow man and killed him. This seems to put a halt to the quest, so we reloaded an earlier save where we had prayed to 4 altars out of the 7.
So we just did the altars again, one near the shadow prince and one of the elves. - And then we also had do a fight again and head to the drowned temple.

We managed to have the quest log thing say we prayed to all altars, ( and all were shining ), but the achievement was still stuck on 5. (yes I refreshed the achievement page)

We did have a couple mods ( AcademyLessonsForAll, BarteringTweaks,LeaderLib,WeightTry2), but none of those apply and it would make no sense we would make progress, but stop making progress suddenly in a quest because of a mod that has no reason to change anything on Nameless Isle.
The gift bags we have enabled are: Source Meditation, Animal Empathy, Enhanced Spirit Vision, Fort Joy Magic Mirror, Improved Organisation, Endless Runner.
We also enabled an achievement enabler, as we felt that the mods we had shouldn't disable achievements that have nothing to do with the mods. ( it worked on everything so far, including Technical Eggstacy).
We're also playing on the newest version of the game. ( no update on steam available)

I also made a save just before getting the 7th altar. That save can be downloaded HERE.
Do note, our inventory is FULL with a lot of stuff so it's going to take quite a bit for the save to load.

Unfortunately, I believe the Achievement mod is incompatible since the armour DLC was released a couple of game updates ago; there have been several reports of it no longer working.

Was the counter going up in your current playthrough, or could that have been from a previous one, if applicable? Or did you take a break before the Jun 14th update and recently return to the game?

When I tested the achievement some time ago, the counter usually didn't go up until I exited Steam and restarted (in once case it went up after a few minutes, while I was doing something else in-game), though the achievement itself unlocked immediately. When there was a problem with the achievement in the Definitive Edition, a couple of people that played the game in the original release reloaded a Nameless Isle save from their earlier playthrough and got the achievement in the Classic version.

EDIT: see the reply below for (possibly) an easier fix.

Try creating a new profile, than copy a save from the start of the Nameless Isle there. If you load the save without any mods enabled, hopefully that will let you get the achievement, if it is just a case of the achievement mod no longer working. If there are later achievements you still need to get, you may want to try disabling mods in your existing profile, and continuing without them.
Each save is a folder in the '..\Documents\Larian Studios\Divinity Original Sin 2 Definitive Edition\PlayerProfiles\<ProfileName>\Savegames\Story' folder.

There is a D:OS 2 save editor which has a function to remove mods from saves, which might help (I have not tried it), and you can add some boosts to a piece of equipment your characters can use temporarily to speed up the required fights (having already gone though some of them twice), and if possible lower the difficulty temporarily, etc.

Norbyte's Script Extender works in the newest patch to re-enable achievements with mods active. It also has other benefits such as reportedly speeding up load times.

Thanks for the correction. Someone who contacted support a month ago reported that Norbyte's Script Extender didn't help, and I have not been following updates if this was added/changed since then.

I'm just going by chatter I saw in Discord only a few days ago, and haven't seen complaints about it since so it's only an assumption. Could still be wrong though, so please let us know if it doesn't 😊

I am using the script extender if I recall correctly, and the achievements do unlock prior to this achievement and after this achievement. Only this achievement doesn't seem to work for some reason.

So I did the profile trick, loaded the save with all 7 altars prayed to, and then prayed to the final altar. It did not unlock the achievement, but now the achievement is at 6/7.
So I have 2 assumptions right now why it doesn't work properly:

1. Coop activating the altar does not increase my progress with the achievement.
2. Altar achievement didn't work for some reason before, and now it does unlock a certain altar somehow. ( but still not the achievement when quest progress is 7/7 )

So to fix this I'd probably need to pray at every altar before having them activated and hope I get the right altar. I really don't want to do that though. - Even with someone else his save file, I'd need a save file of every altar unactivated most likely.

I can confirm that in coop, you have to personally activate every altar to get the achievement. I recently did this myself with the script extender and if my friend prayed at one, I did not get credit for it. With a fresh playthrough, praying at them all myself got me credit.
