1) An item that has the picture of a ring is labelled as 'bone sword', and is in fact a weapon.
2) Lots of keys in Act 1 dungeon 2 that appear to have no use
3) In Act 1 dungoen 2 there is no boss in the 3rd floor, instead there are TWO bosses in the first dungeon 3rd floor.
4) Sometimes enemies will be in the black space outside the map.

These were all in 1.4, and some of them even in 1.0. That I know of they've all been reported, too. And the boss thing, that's just one of the generation possibilities.
