I have seen this as well. The most extreme version was when the goblins attack the tieflings at their camp when you meet them. One of the defenders was killed by a goblin and their model went through a crazy tumbleweed-in-place ragdoll before stretching out in an opaque black plane that bisected the area. The camera couldn't see any texture behind the plane. If I had to describe what it looked like, it was as if you took a shoebox diorama and inserted a large, extremely thin bit of intangible, completely opaque black glass at about a 45 degree angle that hid the back of the box. It disappeared after a save and reload.
