What does the colour of the name of the item mean? Some are written in yellow, some in orange, some are green and labelled unique, some are white and labelled unique...

I think, the normal ones simply imply how valuable the are:
Best is orange, then yellow, green, white. Special/unique/set items are of a different colour to mark them.

I go for orange, if the stats are good - and in one case: for a set-item. Some of them are indestructible.

Sadly, a set can never be complete, as they are strewn throughout the acts and lose their set-item character after changing an act.

Errhm, to quote out of my memory:
"It is difficult to keep the sets status from act to act."

I believe, it was a Larian decision - but this is my interpretation only. They don't change their stats, only the status as a set item.

What's the point of having sets then? It's a bug.... if they keep their stats can't the game just rename the item to the correct thing at the start of the new act.... and usually the purpose of sets is, once you've got the whole lot, you get some new bonuses.
