I was running around the Underdark and I came a cross a area when I tried to climb up the vines my character would begin and then let go and fall through the floor and die.

There is another area like this with a ladder leading up to a crate and a bottle of wine, the platform cannot be teleported to, jumped to, or reached with the provided ladder.
