After refusing Ethel's first offer within her home, I talked to her again and choose the attack command. She then ran away after calling for help. Once I got to her again at the end of the dungeon she still had the cut-scene for when you get close to the cage, but she never attacked me despite being hostile, and I could then wail on her until she was dead.
