I saw a ledge across a river in a crypt and I used Misty Step to reach it. I'm guessing it was out of bounds? Unable to move/jump and was half buried in stone: https://imgur.com/a/aZaG4yc

Fell into the Underdark via phase spider cave under the Blighted Village. Camera got stuck and lost my UI on the way down: https://imgur.com/a/P3tZNdk

While battling the mama Owl Bear in her cave the cutscene activated when she used Owlbear Rage. After the cutscene it was just a blank screen, although I still had control and could continue, just blindly. Could not get it to repeat upon reload. No screen shot here, because it was just a black screen. Just close your eyes, you'll get it =D

I noticed the number indicating how many superiority die you have left as a battle master doesn't update as you use them. It always says 4.
