For me middle mouse camera rotation randomly stops working - doesn't help if I reload, quit game, try to change key bindings...
After a while (usually some cutscenes - scripted, just talking to companions won't do it) it also randomly returns.
It really bothers me as I hate key rotation especially in combat (well aside from that I haven't experienced many gameplay bugs so well done
)
